What Happens Inside the Body as We Age? The Hallmarks of Aging Explained

We all grow older, but biological ageing is more than the passing of time.

Inside the body, cells gradually collect damage, produce energy less efficiently and become less able to repair themselves. These changes do not happen in only one organ. They affect many tissues and systems at the same time.

In 2013, researchers grouped the main biological changes linked with ageing into nine areas known as the Hallmarks of Ageing.

The idea is simple: ageing is not caused by one single problem. It develops through several processes that influence one another over many years.

In simple terms, the nine original Hallmarks of Aging describe the following changes:

  1. DNA gradually accumulates damage — genomic instability.
  2. The protective ends of chromosomes become shorter — telomere attrition.
  3. The way genes are switched on and off begins to change — epigenetic alterations.
  4. Cells become less able to maintain, repair and remove damaged proteins — loss of proteostasis.
  5. The balance between energy use, growth and repair becomes disrupted — deregulated nutrient sensing.
  6. Cells produce energy less efficiently — mitochondrial dysfunction.
  7. Some damaged cells stop dividing but remain active in the body — cellular senescence.
  8. The body becomes less able to replace damaged or worn-out cells — stem cell exhaustion.
  9. Communication between cells, tissues and organs becomes less coordinated — altered intercellular communication.

These changes do not develop separately.

Damage to DNA may affect how genes work. Less efficient mitochondria may place cells under greater stress. Damaged cells that remain in tissues can release inflammatory signals. Persistent inflammation may then make tissue repair more difficult.

Over time, one problem can place additional pressure on several others.

This helps explain why ageing affects the whole body rather than one organ at a time. It also helps explain why conditions such as cardiovascular disease, diabetes, frailty and neurodegenerative diseases can share some of the same underlying biological changes.

The Hallmarks of Ageing gave scientists a clearer way to study these processes.

Instead of treating every age-related condition as completely separate, researchers could examine the common changes that make many diseases more likely as we grow older.

However, the nine hallmarks are not nine simple medical tests.

They cannot tell one person exactly how quickly they are ageing, and they should not be treated as a score that can predict an individual future.

Their value is that they help us understand the bigger picture.

They show that healthy ageing depends on many connected systems: protecting DNA, maintaining proteins, producing energy, regulating metabolism, removing damaged cells, repairing tissues and keeping communication between organs working effectively.

This also explains why no single supplement or intervention can control the entire ageing process.

Regular physical activity, balanced nutrition, adequate sleep, avoiding smoking and good control of blood pressure, cholesterol and glucose may support several biological systems at the same time.

The aim of longevity medicine is not to stop time or promise that ageing can be reversed.

It is to understand what changes inside the body, reduce avoidable damage and preserve strength, function and independence for as long as possible.
