Health is often described as the absence of disease.
If blood tests are normal, no diagnosis has been made and no major symptoms are present, a person may be considered healthy.
But this definition is incomplete.
The body does not remain healthy by doing nothing. Every day, it must protect its boundaries, remove damaged material, coordinate signals between organs, maintain internal balance and repair tissues after stress or injury.
Health is therefore not a passive state. It is an active biological process.
The Hallmarks of Health framework was created to explain the systems that allow the body to remain stable, adapt to change and recover from damage.
Some of these systems are easy to understand.
The skin, intestinal lining and blood-brain barrier control what can enter different parts of the body. Cells continually remove damaged proteins and worn-out components. The nervous, hormonal, immune and metabolic systems exchange information so that the whole body can respond in a coordinated way.
Timing also matters. Sleep, hormone release, metabolism and immune activity follow biological rhythms. When these rhythms become repeatedly disrupted, different systems may begin to work out of step.
Another essential feature of health is the ability to return toward balance after a disturbance.
Body temperature rises during infection. Heart rate increases during exercise. Blood glucose changes after a meal. A healthy body responds to these challenges and then gradually returns toward its usual range.
This ability to adapt and recover is known as resilience.
A person may have no diagnosed disease but may already be losing part of this resilience. Recovery after infection may take longer. Poor sleep may have a stronger effect. A short period of inactivity may lead to a noticeable loss of strength.
These changes can matter even before a conventional diagnosis appears.
The Hallmarks of Health do not form a medical test, and they should not be treated as eight measurements that can be checked in a clinic. They are a conceptual framework that helps explain what the body must continue to do well in order to preserve health. The framework identifies eight interconnected Hallmarks of Health that together help the body preserve function, respond to disruption and repair damage.
For longevity medicine, this changes the question.
Instead of asking only, "What disease is present?", we should also ask, "What biological capacities are being preserved?"
This includes the ability to maintain muscle strength, recover after physical stress, regulate metabolism, sleep well, coordinate immune responses and repair damage.
No single supplement, test or intervention can protect all of these systems.
Health is maintained through the interaction of many processes. This is also why regular physical activity, adequate sleep, appropriate nutrition and preventive medical care can have such broad effects: they support several biological systems at the same time.
Healthy aging is not simply reaching an older age without a diagnosis.
It is preserving the body's capacity to remain organised, adaptable and resilient throughout life.
